**Registration for the 2009-2010 School Year**



Please bring the following with you and your child:

1. Proof of age: Original Birth Certificate or a Passport

2. Proof of address: Any two (2) of the following:

a. Utility Bill (gas or electric)
b. Documentation or letter on letterhead from a federal, State or local government agency, including the IRS, City Housing Authority, Human Resources Administration or Administration for Children's Service
c. Original Lease Agreement, Deed or Mortgage Statement
d. Current Property Tax Bill
e. Water Bill
f. Official Payroll documentation from employer issued
within the past 60 days (with present address).









3. Proof of immunization: Yellow card filled out by the pediatrician.

4. Physical Examination Form: Must be returned as soon as possible.

5. Child's latest report card

6. Child's Individual Education Program (IEP) and/or 504 Accommodations (if applicable).

7. Your child MUST be present at registration
